Good Faith Estimate

Beginning January 1st, 2022, under the No Surprises Act, healthcare providers must give clients who do not have insurance, or do not plan to use insurance, an estimate of the expected charges for medical services, including therapy services.

This new regulation is designed to provide transparency to patients regarding their expected medical expenses and to protect them from surprises when they receive their medical bills. It allows patients to understand how much their health care will cost before they receive services.

There are a number of factors that make it challenging to provide an estimate on how long it will take for a client to complete therapeutic treatment. These factors depend on the individual client, the presenting issues, and the agreed upon treatment plan. Ultimately, as the client, it is your decision when to stop therapy.  Before our first appointment, I will discuss a Good Faith Estimate with you and will provide you a written copy.

 The Good Faith Estimate is not a contract between provider and client and does not obligate or require the client to obtain any of the listed services from the provider.  If your bill for services is $400 or more than your Good Faith Estimate, you have the right to dispute it.

Payment & Cancellation Policy

Payment is due at the time services are rendered.

Accepted payment methods include major debit and credit cards, as well as eligible FSA and HSA cards.

Insurance

This practice does not bill insurance directly. Clients may seek out-of-network reimbursement independently if eligible.
Upon request, the practice can provide a monthly Superbill (a detailed receipt of services rendered).

Cancellation Policy

Appointments canceled or rescheduled with less than [48] hours’ notice may be subject to a cancellation fee.
Missed appointments may be charged the full session fee.

Cancellation and missed-appointment fees are the responsibility of the client.
